What is the MCA change request form?
The MCA change request form is a web form on the V3 portal for a correction that cannot be made with an existing e-form. It covers an error in master data, and a direction of a court or tribunal for which the portal has no other service. It is examined by an officer. Filing it is not the same as filing the form the Act already provides.

What can it correct?

What should still use its own form?
- A director’s appointment or resignation is DIR-12, within 30 days.
- A company’s registered office is INC-22, within 30 days.
- An LLP’s registered office is Form 15.
- The annual accounts and the annual return stay on their own forms.
Using the change request form for one of those events does not satisfy the section that requires the specific form.
How is it filed?
Sign in on MCA V3, open the Change Request Form under the Registrar’s approval services, enter the company or LLP, and sign the form with the digital signature. The portal generates an acknowledgement. The acknowledgement records that the request was received.
Who decides it?
The circular asks the Registrar to process the form and send it to the Joint Director, who decides the request. The circular’s timeline is three days with the Registrar and seven days with the Joint Director. A request can be sent back if the correction can be made on an existing form, or if the attachment does not support the change.
